All audit details and requirements will be communicated to the Sponsor Representative and/or Primary Administrator. All personnel are asked to cooperate with the auditor. All documents requested prior to the audit must be made available for the audit.



For Accredited Training Sponsors

  • Registration & Release Forms
  • Module Completions (written and performance end-of-module tests)
  • Instructor Records (Form 101, Form 702A, Instructor Certification Training Program (ICTP) written and performance tests, documentation of qualifications, and evaluations)
  • Equipment
  • Training Facilities¬†
  • Observation of a training session


For Accredited Assessment Centers

  • Registration & Release Forms
  • Current Assessment Inventory Log/Spreadsheet (503A) - if paper assessments are used
  • Performance Verification Submission Sheets
  • If paper assessments are used, auditor will ensure demographic, answer, and security statement sheets have been maintained.
  • Authorized Assessment Site audits
  • Coordinator and Proctor Records (Form 101A, Form 703A, certification assessment, and evaluations)
  • Performance Evaluator Records (Form 101, Form 702A, Instructor Certification Training Program (ICTP) written tests, documentation of qualifications, and evaluations Testing Area)
  • Primary Administrator will administer a proctored assessment session to the auditor using a Sample assessment
  • Equipment and Performance Verification (PV) facilities (if applicable)

For Mobile Crane Certification Programs

  • All equipment specified on the Mobile Crane portion of the Form 900 (initial audit only)
  • Observe any of the four practical exams (Rubber Tire Truck Mount, Rough Terrain/All Terrain, Industrial/All Purpose, or Crawler Mount)

For Rigger and Signal Person Certification Programs

  • All equipment specified on the Rigger and Signal Person portions of the Form 900 (initial audit only)
  • Observe an Advanced Rigger and Signal Person Practical Exam
